- up to $3m of Death insurance cover  and $2m of Total and Permanent Disablement cover
- income protection cover up to a maximum of 85% of your taxable income (includes a monthly superannuation payment of up to 10%)
- income protection maximum weekly benefit of $4,600
- choice of 3 income protection benefits payable for a maximum of 2 years, to age 60 or to age 65.

2)  Death & Total and Permanent Disability Cover – complete this change of membership details form
