The pack contains the form you will need to complete as well as information and forms that you will need to supply to your employer.

The pack includes:

1) a guide for your employer to set up their account and make contributions;
 
2) a standard choice form on which an employee can nominate legalsuper as their chosen fund;
 
3) a letter confirming legalsuper's complying fund status and how an employer makes contributions to legalsuper; and
 
4) an employer application and initial contribution schedule.
